Feedback & incident/injury report

Seattle Pride Hockey Association is committed to fostering a safe, inclusive, and respectful environment for everyone. If you are a player, spectator, volunteer, or supplier who has experienced or witnessed an incident / injury OR wish to provide feedback regarding our programs separate from our program surveys, we encourage you to complete the report below. Your report helps us research & investigate the event, address potential risks, and improve the safety and integrity of our programs.

For players who sustain an injury on the ice during a sanctioned USA Hockey program (such as the Seattle Pride Classic, Frozen Pride Classic, Seattle Pride Hockey Training, or Seattle Pride Hockey League), this report is secondary to the claims process required by USA Hockey and does not replace it. All reports are submitted to and reviewed by our Conduct & Ethics Committee.

This report serves a dual purpose: to document both incidents/injuries incurred during a program and to collect feedback related to the operation of our programs. Below are examples of what may be reported—though this list is not exhaustive. If your exact situation isn’t listed, please select the most relevant category.
INCIDENT/INJURY FEEDBACK
  • Injuries that initially seem minor but later require medical attention
  • Minor injuries requiring additional aid or care (e.g., cuts, bruises, sprains)
  • Substantial injuries that result in hospital transport
  • Concussions or suspected concussions
  • Equipment-related injuries, such as skate cuts or gear malfunctions
  • Heat exhaustion, dehydration, or other exertion-related conditions
  • Slips, falls, or facility-related injuries
  • Witnessing any injury sustained by another player or spectator
  • A relatively minor event that is incidental to, or related to, other occurrences
  • An event that causes or has the potential to cause a crisis or interruption
  • Damage to or theft of personal property or that of another individual
  • Altercations (physical or verbal) between players, spectators, officials, suppliers, staff and/or volunteers
  • Witnessing or experiencing unsafe playing conditions, such as hazardous ice, broken equipment, or facility safety concerns
  • General program feedback separate from program surveys
  • Violations of SPHA’s Mission or Guiding Commitments (Values), Fair Play Code, Tournament Rules, Participant Conduct, Inclusion in Hockey, League Guidelines and/or other policies/rules (i.e., USA Hockey)
  • Discrimination, harassment, or verbal abuse, including slurs and hate speech
  • Violation of a person’s rights, character, feelings, or interests
  • Misconduct or issues by team captains, referees, scorekeepers, coaches, staff and/or volunteers
  • Facility-related feedback, incidents, or issues
  • Anything that is not an incident/injury to oneself or another person
