Seattle Pride Hockey Association, Inc. (herein after referred to as “SPHA”) respects your concerns about privacy. This Privacy Statement applies to personal information we collect including information collected on the websites and mobile application (collectively, the “Sites”) listed here.

• Forms submitted through our website but hosted on third party websites (i.e., JotForm).

• Registration sites used for SPHA events, programs and/or tournaments (i.e., DaySmart Recreation and LeagueApps)

By visiting the Sites and providing personal information or otherwise providing requested personal information, you consent to the collection, storage, and disclosure of your personal information as described herein. This Privacy Statement describes the types of personal information we collect, how we may use that information and with whom we may share it. The Privacy Statement also describes the measures we take to protect the security of the personal information. We also tell you how you can reach us to ask us to update your preferences regarding how we communicate with you or answer any questions you may have about our privacy practices.

INFORMATION YOU PROVIDE DIRECTLY TO US

Other than as specified herein, all personal information provided by you will be provided with your specific knowledge and consent. The personal information you may provide to us may include:

• Contact information such as name, postal address, telephone number, and email address

• Date of birth, gender, and age

• Payment information, such as credit card number and expiration date

• Information to identify sweepstakes, contest entrants, and notify prize winners

SWEEPSTAKES & COMPETITION CONTESTANTS

Sweepstakes and Competition entrants may provide personal information via the sites or otherwise. By entering SPHA sweepstakes or competitions and providing requested entry information, you are consenting to the collection, storage and disclosure of personal information and the transfer of intellectual property ownership to all materials created as part of the sweepstakes or contest entry to SPHA. Personal information may be collected by third party service providers with whom SPHA contracts to provided services in connection with sweepstakes or competitions and if such is the case, we will request that such service providers make the personal information available to SPHA for our use in accordance with the terms of this Privacy Statement.

Prize winners may be required to provide additional personal information which may be subject to additional or separate privacy terms to be provided when such additional information is requested. Such privacy terms may be those of SPHA, the prize provider or administrator or all three. Acceptance of a contest prize constitutes permission for SPHA and its agencies to use winner’s name and/or likeness and biographical materials for advertising and promotional purposes without additional compensation, unless prohibited by law. Notwithstanding the foregoing disclosures applicable to prize winners, without the prize winner’s prior permission, we will not share a prize winner’s social security number, or other tax or medical information or driver’s license data, or other information that may be subject to privacy protections under federal and state law, if such information is collected as a condition of receiving a prize.

INFORMATION WE COLLECT BY AUTOMATED MEANS

When you visit our Sites, we collect certain information by automated means, using technologies such as cookies, Web servicer logs and Web beacons.

You may also unsubscribe from promotional emails by clicking on the “unsubscribe” link in any promotional email you received from us. If you “unsubscribe” from all email communications from SPHA, you may be unable to claim a prize in a sweepstakes, contest, or competition, where notification to winners is made exclusively by email.

HOW WE PROTECT PERSONAL INFORMATION

We maintain appropriate administrative, technical, and physical safeguards to protect the personal information you provide on our Sites against accidental, unlawful, or unauthorized destruction, loss, alteration, access, disclosure or use and other unlawful forms of processing. Seattle Pride Hockey Association takes no responsibility in data breaches and/or data loss by third party vendors.

PROVIDING INFORMATION THROUGH THIRD PARTY SITES

Our Sites may provide links other websites for your convenience and information. Those websites may be operated by entities not affiliated with us. We have no control over and assume no responsibility for the content and privacy policies or practices of any third party including, but not limited to, social media sites and user generated content sites. Linked and other third-party websites may have their owner privacy policies which we strongly suggest you review if you visit and provide personal information intended for use by SPHA through any linked websites, social media sites or other third-party sites.

CHILDREN’S PRIVACY

The Sites are not intended or directed to children under the age of thirteen and we do not knowingly collect personal information from children under the age of thirteen on the Sites. If we are notified that we have inadvertently received personal information from a visitor under the age of thirteen on the Sites, we will delete the information from our records.

UPDATES TO OUR PRIVACY STATEMENT

This Privacy Statement may be updated periodically and without prior notice to you to reflect changes in our personal information practice or relevant laws. We will post a prominent notice on spaceneedle.com to notify you of any significant changes to our Privacy Statement. We will alert you that changes have been made by indicating on the bottom of this Privacy Statement the date it was last updated. These disclosures, terms and conditions may be modified from time to time to be consistent with applicable federal and state law.
